Use this form to report any missing physical materials, or other materials you were expecting and have not yet received. If you are having problems accessing any online tools, FIRST submit a helpdesk ticket to IS&T, and if it is not resolved, THEN use this form to alert us of your issue. Please report only ONE issue in each form. You can complete this survey multiple times if you have multiple issues. This form is for internal Lawrence Public Schools use only.
